We’re looking for a Care Coordinator to join our brilliant team in Mid-Cheshire. This crucial role ensures our clients receive exceptional care. The role involves organising schedules based on clients' requested call times and care professional availability, as well as maintaining effective communication with staff, clients, families, and professionals. This is a service-based role, and you will be expected to provide hands-on care support when required to ensure continuity of service. Key responsibilities Create and maintain weekly care schedules, ensuring all client visits are covered efficiently Manage day-to-day scheduling changes and respond quickly to unforeseen circumstances Managing staff absence, allocating calls, and cancelling care calls when necessary Assisting in the training and induction of new team members Allocate care calls and arrange cover where required Conduct care visits and provide field-based support as needed Maintain regular communication with clients, families, and healthcare professionals Ensure accurate record-keeping and compliance with company policies and procedures Help deliver a reliable, responsive, and high-quality care service Participate in the out-of-hours on-call rota when required About you We're looking for someone who is Highly organised with excellent time management skills Experienced in care coordination, scheduling, or care service delivery (desirable) Passionate about providing outstanding care and customer service A confident communicator who can build positive relationships with people at all levels IT literate and comfortable using scheduling and care management systems Calm under pressure and able to solve problems effectively Reliable, proactive, and able to work independently A positive team player who helps motivate and support colleagues Committed to personal and professional development Essential requirements Full UK driving licence and access to a vehicle Flexibility to provide care support when required Enhanced DBS check (or willingness to undertake one) Right to work in the UK What we offer Full-time permanent position Competitive salary of £26,000 to £28,00 (depending on experience) 20 days annual leave plus bank holidays Paid mileage for business travel Paid induction and ongoing training Career progression opportunities within a growing organisation Supportive, professional and approachable management team Blue Light Card discount scheme Paid DBS check Employee recognition and reward programmes A positive workplace culture where your contribution is genuinely valued If you're ambitious, compassionate, and looking to build your career with a quality-focused home care provider, we'd love to hear from you.
